This building at 2866 Center Street NE on the Oregon State Hospital campus was originally built in 1937. Also known as Building #49, it was previously used as the Salem Rehabilitation Facility.

This building was scheduled to be demolished to make way for the new construction at the Oregon State Hospital. Demolition began on March 5, 2010.
